
Department VLAN Worksheet - Instructions for Completion
- Department and technical contacts need to designate how many VLANs they want to have within their department using the ‘Department VLAN Worksheet'.
- List the VLANs in a numerical sequence beginning with one (1). The Network 21 project can accommodate 300-400 VLANs. In a majority of cases a department should only need one VLAN. Given that there are 255 departments included in the project, departments should try to limit their VLANs to one or two.
- Complete the ‘Description/purpose of the VLAN'. There is no required format for this information. However, when a VLAN encompasses multiple departments, the associated departments must coordinate and decide upon a consistent VLAN description.
- Complete ‘Primary Department Owner' for the VLAN, usually your department. This should follow the standardized department title format used on the attached worksheet: "Standard Department Names for VLANs". On any given VLAN only one department can be the Primary Department Owner.
- Complete ‘Other Depts on VLAN' only if other departments are part of your VLAN, or if you are part of another department's VLAN, otherwise leave blank.
- When designing VLANs that encompass multiple departments, the associated departments must coordinate the VLAN numbering sequence. Do not reuse VLAN numbers that have already been used to identify a shared VLAN. The worksheet should be filled out on the basis of this coordination. Use the standardized department title format from the attached worksheet: "Standard Department Names for VLANs" when choosing department names. When there are multiple departments associated with a particular VLAN, draw a downward pointing arrow in the Primary Department Owner column through the last row that corresponds to that VLAN.
The following example shows how to fill out the worksheets for a shared VLAN:
The first VLAN is shared with Information Resources and Communications Resources. Information Resources, using the Standard Name format (IT INFO RES), is indicated as the ‘Primary Dept Owner' for VLAN#1. Communications Resources, using the Standard Name format (IT COMMUNRES), is indicated as ‘Other Depts on VLAN'.
The second VLAN's only member is Information Resources. The Standard Name format (IT INFO RES) is indicated as the ‘Primary Dept Owner' for VLAN#2, and ‘Other Depts on VLAN' has no entry for that particular row.
Information Resources will complete their VLAN Worksheet as follows:

Communications Resources completes their own worksheet and also follows the Standard Name format when choosing departments. For VLAN#1, Communications Resources indicates IT INFO RES as ‘Primary Dept Owner' and IT COMMUNRES as ‘Other Depts on VLAN'. For the remaining two VLANs, used by the business office and service center, respectively, IT COMMUNRES is indicated as ‘Primary Dept Owner'. The corresponding rows in ‘Other Depts on VLAN' is left blank because there are no other departments on those VLANs.
Communication Resources will complete their VLAN Worksheet as follows:

NAM Verification Section 1. NAM Verification
- The NAMs listed in section 1 of the ‘NAM Verification Worksheet,' are the NAMs currently scheduled for Network 21 Stage 3 Cutover. Verify the NAM information is accurate. The information is critical to the network design and cutover process. The more accurate this information , the less downtime your department will experience during cutover.
- Indicate which NAMs are no longer needed by checking the ‘NAM Delete?' column. It is not necessary to make any marks if the information is correct.
NAM Verification Section 2. VLAN Information to Supply
- Indicate which NAMs connect workstations or printers that use the listed protocols by checking the appropriate column. The choices are Apple, DecNet and IPX. This information is needed for initial implementation only and is not necessary each time a device is moved from one NAM to another.
- The NAMs listed in section 1 of the ‘NAM Verification Worksheet,' must be assigned a VLAN number, depending on the configuration desired by the department. Write the corresponding number in the ‘VLAN # for this NAM' column for each of the NAMs that are to be connected to the network. Refer to the Department VLAN Worksheet for the VLAN #.
- Write the appropriate number of devices connected to the NAM in the ‘# Devices to be served by NAM' column. When determining the number of devices serviced at each NAM, consider any hubs with multiple attached devices, connected to a particular NAM.
- To complete the survey and authorize your NAM and VLAN information, please provide the signature of the department or technical contact.
- Return your completed survey via campus mail to: Network 21 - Communications Resources. The survey must be received as soon as possible.
