Service Order Requests

Submitting Service Order Requests:
Service Order Requests must be submitted by the department signature
authority.
There are several methods available to submit a 'Service Order Request'.
In your request, be sure to include area, department, building, room,
NAM#, Billing ID, and the change. If there are questions, please call 752-4603.
Ordering a NAM activation for immediate
network connectivity:
Requests may be submitted if your department has an existing local
area network.
Current rates:

$60.00 NAM activation
-
activations are applicable when connecting a NAM to a 10BASE-T port
(assumes a 10BASE-T ready NAM exists)

includes connecting the NAM to an existing hub and testing the
connection.

$400.00 NAM installation
-
for each additional NAM to be installed
-
$200.00 Network 21 port cutover (per port)

includes this new connection in your department's Network 21
cutover

Monthly Rates:
The approved subsidized local electronics rate of $4.00/month per networked connection for I&R funded departments, and the non-subsidized local electronics rate of $24.54/month per networked connection for Non-I&R funded departments.
Requests for Local Area Network (LAN) installations, or installation of
additional hubs, requires an estimate and is based on a time and materials
charge.
